Four hospital staff failed to spot overdose given to boy 5

A five-year-old boy died after hospital staff administered a massive overdose of medication, a coroner said. Bailey Ratcliffe, from Batley, West Yorkshire, died in May 2009 after nurses and doctors gave him 11 times the appropriate dose of phenytoin, an epilepsy drug, causing his heart to stop. His inquest at Bradford Coroner’s Court was told how staff at Dewsbury and District Hospital made catastrophic mistakes when calculating the dose of the drug, even though four of them were
